Cardinal McCloskey's Service Coordination Program provides case management services ensuring that more than 65 developmentally disabled adults receive the vital services they need to help them reach their full potential.
Comprehensive Coordination and Referral
Most services to individuals with mental retardation are provided through Medicaid funding. Our staff of Service Coordinators works with individuals and families to evaluate and decide which services the person needs and then provides assistance in accessing these services. Accessing governmental benefits is often one of the first steps. Accessing other services includes locating programs or services to provide medical care, clinical care, social services, educational assistance, vocational training, financial assistance, residential support, employment services as well as to meet any other special needs or requirements.
Based upon each individual's strengths, abilities and desires as well as family needs and personal preferences, our Service Coordinators advocate for rights to service and then collaborate the critical services. Once services are being provided, our Coordinators monitor the service delivery to ensure it continues to meet the needs, making appropriate changes when needed.
Trained & Experienced Staff
The CMS Service Coordination department is comprised of highly experienced Service Coordinators and a Service Coordination Supervisor. All have participated in specialized training regarding funding/service coordination updates and mandates.
Services & Activities
Our Service Coordination Department is responsible for:
- Service Planning including establishing a written Plan of Service to meet each individual's functional status, service needs as well as personal goals and preferences
- Linkage and referral to ensure access to identified and chosen services
- Service Coordination including coordinating, developing and participating in service plan assessment meetings with service providers
- Maintaining on-going contact with the individual and family to ensure on-going delivery of services
- On-going evaluation to determine if services and supports are appropriate to the individual's needs, desires and goals
- Advocacy to protect each individual's rights
